Hace 5 años aproximadamente me diagnosticaron una enfermedad genética llamada MEN2A, la cuál produce cáncer de tiroides, y por otra parte feocromocitomas en las dos glandulas suprarenales. Desde el primer dia el doctor Oscar Vidal se reunió conmigo y mi família para informarnos de forma muy clara e incluyendo los posibles riesgos de la enfermedad y de las operaciones quirúrgicas, lo que se agradece mucho cuando te diagnostican una enfermedad. La primera operación de la tiroidectomia total (2015) fué un éxito, rápida y sin marcas visibles. La segunda en la cuál me extirparon las dos glándulas suprrarenales a la vez, realizada en el mes de Junio de 2020, fue de nuevo otro gran éxito por parte del doctor Oscar Vidal. La experiencia fue inmejorable. Fui operada por El Dr. Vidal el pasado 5 de Julio de un quiste en la suprarrenal derecha de 5,5cm, el doctor me comentó antes de la operación que intentaría dejar algo de glándula y no extirparla entera, y así lo hizo, ha dejado 2/3 de la suprarrenal y la operación se ejecutó en una hora. Las cicatrices son mínimas y la recuperación fue muy rápida y sin dolor. Además tengo que resaltar también, la profesionalidad del Hospital Clinic. Estuve debatiendo si operarme con el Dr. Vidal desde el privado o desde Hospital Clinic y me alegro mucho de haber tomado la decisión correcta a pesar de tener que hacer la lista de espera. Vidal me comentó que si existía algún problema en este hospital tienen todo lo necesario para abordarlo, y esto se podía reflejar sin duda en los modernos quirófanos. Con respecto a su personalidad y trato al paciente, las citas anteriores a la operación puede que observara un poco de frialdad, pero sin duda estaba siendo un gran profesional, es más, me hizo algunas recomendaciones ya no como médico sino como si se tratara de un familiar, por lo que esto dice mucho como persona. Y hasta la última visita después de la operación se mostró con mucho humor. Busqué varios doctores al principio, pero no me dieron la confianza que Vidal me dio. Sin duda, mi mejor recomendación para esta intervención. Gracias doctor.

On September 10th, Dr. Oscar Vidal attended the Rey Juan Carlos University Hospital to participate in the Interdepartmental Meeting organised by Prof. Manuel Duran. The aim of the meeting was to share the joint strategies to be followed in the area of Endocrine Surgery between the aforementioned hospital and the Clinic Hospital of Barcelona. Vilaça organised and co-directed an international day on the advances in the diagnosis and treatment of thyroid diseases, which included the most relevant world-renowned specialists in the field of thyroid surgery. On May 25th, World Thyroid Day, Dr. J. Dr. Òscar Vidal and his team performed the first endoscopic thyroidectomy performed via an axillary approach of the Hospital Clínic of Barcelona.

This has been possible thanks to a project developed jointly with other hospitals in the world, with special mention to the Da Luz Arrabida Hospital in Porto with its leading surgeon Dr. Jaime Vilaça, with the purpose of improving and innovating in this field. Specific training has been carried out, first on laboratory animals and later on cadavers in order to consolidate a new approach that will provide adequate levels of comfort, quality and safety for patients affected by thyroid nodules, without having to make neck incisions.

En octubre/noviembre de 2015 empecé a notar calores y cansancio porque tenia la presión arterial muy alta, llegué a tener mas de 200. Me hicieron pruebas de todo tipo y al final me diagnosticaron feocromocitoma en las dos glándulas suprarrenales. Después de meses con calores, dolores de cabeza fuertes y mucho cansancio, me operó Dr. Vidal en Barcelona en junio. Me las extirparon enteras. Mi primera operación, estaba muy asustada, pero resultó que no lo pasé tan mal como pensaba. No me enteré de nada. Después de la operación me tuvieron en la uci un par de dias para controlar mi presión arterial, pero nada más operarme bajó, la tenia normal a 90 más o menos. Al siguiente dia de la operación yo me encontraba bien, solo sentia en la barriga como si tuviera agujetas de reirme o de hacer abdominales. No sentia dolor la verdad, solo esa pequeña molestia. El tercer dia me pasaron a planta normal y al dia siguiente me vieron ya perfecta y me enviaron para casa. Me mandaron dos pastillas que tengo que tomar ya de por vida porque no tengo suprarrenales(Astonin e Hidroaltesona) A los diez dias yo ya salia a pasear, quedaba con la gente.. La barriga la tenia hinchada pero con los dias se me iba deshinchando. Las heridas cicatrizaron bien y rápido. Tengo tres pequeñitas en cada costado. A las dos o tres semanas ya iba a la playa, salia por la noche, paseaba... bueno vida normal. Y bueno en octubre ya empecé a trabajar otra vez, luego en enero me apunté al gimnasio, hago deporte, he engordado 14 kilos desde antes de la operación, antes de operarme pesaba 46 y ahora 60 y estoy muy bien y feliz. Agradecerle a Vidal por el trato y la profesionalidad, me siento estupendamente desde hace meses.

This cancer can occur in any person and even in a previously healthy thyroid gland, it is known that between 5 and 15% of the thyroid nodules are potentially malignant. Hola, mi nombre es Martin Arcadio y a principios de 2016 fui diagnosticado de una enfermedad endocrina llamada MEN2A, y eso conlleva a cáncer medular de tiroides y feocromocitomas en las glándulas suprarrenales, la verdad es una palo grande, pero otra gran verdad es que a día de hoy tenemos a gente muy y muy profesionales, y uno de esos ejemplos es el Dr.Oscar Vidal, que decir... solo que gracias y muchas gracias por estas dos operaciones, seguramente note van a idolatrar como a un gran futbolista, pero ahora a día de hoy en mi casa eres nuestro Héroe. Dr. Oscar Vidal was invited as a professor to this prestigious Endocrine Surgical course for specialists in adrenal surgery. The course, directed by Professor Juan Manuel Martos, was organised at the Surgical Department of the Virgen de Rocio University Hospital in Seville on July 6th and 7th. It was an eminently practical day in which Dr Oscar Vidal gave a lecture on the latest developments regarding the surgical treatment of adrenal gland tumours and successfully completed a left adrenalectomy by retroperitoneoscopy. Some of the assistants were also able to take part in the surgical procedure.

El 30 de noviembre de 2015, hoy justo 5 meses de mi intervención, tuve la gran suerte de haber encontrado este excelente profesional.Tanto el Dr. Vidal como su equipo y el personal de la clínica Quirón brindan un trato único. Com pacient li vull agrair, especialment, al Dr. Vidal dues coses : En la consulta prèvia i el preoperatori rebre una informació exacta, estructurada i realista que va fer augmentar la meva seguretat i confiança en el procés. Després, durant l'ingrés i la intervenció (que suposo molt complexa), la sensació personal de quasi no haver patit un procés quirúrgic. L'estada clínica va ser inferior a 24 hores i, a partir de l'alta, no em va caldre cap tipus d'analgèsic, ni fer cap règim alimentari especial. Vaig tenir sensacions de dolor molt baixes. Només vaig haver d'evitar esforços físics i vocals, dins d'un context de vida quotidiana normal.

Bona nit, Fa aproximadament mig any vaig anar a veure al dr. Vidal per primera vegada. M'hi va enviar l'endocrí ja que, després d'haver-me sotmès fa anys a una tiroidectomia parcial, en l'actualitat no em funcionava bé la tiroides i la tenia plena de quists. Només veure les meves proves i resultats em va dir quan t'operes, d'aquí 15 dies? Em va agafar en fred, però ell va dir si una cosa s'ha de fer no cal esperar. Em va donar tanta confiança i m'ho va explicar tot tan bé que al cap d'uns dies vam acordar el dia de la intervenció. Ara ja han passat 5 mesos, i vull donar les gràcies al doctor per ser tan professional, però a més, i molt important, per ser tant proper i donar aquell suport que et fa falta quan tens por. Quan topes amb un problema de salut el primer que vols és caure en mans d’un bon professional amb vocació, que tingui un tracte proper amb el pacient i que t’expliqui de forma clara i senzilla la teva situació. Quan vaig anar a la primera visita amb el Dr.Vidal ja tenia opinions d’altres especialistes. En la meva situació no se sabia a ciència certa si s’hauria de fer una tiroïdectomia subtotal o total. He de dir que em vaig topar amb un gran professional, vaig rebre un tracte cordial i proper, va ser molt sincer i clar amb el diagnòstic i em va transmetre tranquil·litat i confiança. Vaig sortir de la consulta amb les coses clares i sabent el que havia de fer. A dia d’avui ja estic operada pel Dr.Vidal. Finalment va ser una tiroïdectomia subtotal. El doctor va seguir les directrius que havíem parlat anteriorment a la consulta i tot va anar molt bé. Durant el postoperatori he evolucionat bé, amb tots els símptomes normals després d’una intervenció de tiroides. Estamos en buenas manos!!!!, Antes de visitar al Dr Vidal fui a otros especialistas y ninguno de ellos me explico de manera tan clara lo que me sucedía, cosa que agradecí profundamente. Una vez superado el shock pues no era consciente de la gravedad, me explico como actuar sin ir más allá, sin apresurarse. La intervención fue genial, rápida y el post operatorio igual, el doctor te tranquiliza, sabe que decirte para que vayas relajada... Al menos conmigo lo logro!!! Pasados cuatro años de visitas de médico en médico nos recomendaron ponernos en manos del Dr. Vidal, desde luego fue una gran acierto. The procedure is carried out with general anesthesia by means of a horizontal incision (cut) of about 5 cm in the lower part of the neck. The operation will usually last less than two hours.

Very occasionally, it is necessary for the patient to leave the operating theatre with a drainage tube in their neck.

No operation is without any risk, and although most patients recover completely within a few weeks, the possible side effects must be taken into account.

General risks: Infection, transient and symmetrical twitching and cramping (hands and mouth).
Serious risks: Hematomas in the neck, permanent alterations of the voice or of the calcium metabolism.

Es, sin duda, un modelo de profesional del S. XIX. MUY RECOMENDABLE: trato óptimo, cirujano exquisito. Una persona de trato muy cercano, y afable, además de demostrar una adecuada empatía: ofrece la información que uno precisa (nunca más de la necesaria, esto me parece muy importante, personalmente) para ayudar al paciente a tomar una decisión ponderada y adecuada. De manos impecables, pese a la complejidad NI dolor, NI disfonía, cicatrización ÓPTIMA, estoy incluso sorprendido tanto del postoperatorio como del resultado (Cicatriz). Este mes de Enero me realize una adrenalectomia por laparoscopia para extraer una glandula suprarrenal que provocaba un Sindrome de Cushing. El doctor Vidal me realizó una descripción muy clara de la operación, con todas las posibles complicaciones, pero en ningun caso infundiendo miedo La operación resultó perfecta, y la recuperación fue muy rapida, con un solo dia de hospitalizacion y analgesicos. En todo momento el doctor resulto muy cercano, tanto conmigo como con mis familiares En resumen, una explicación clara y precisa, y una operación y seguimiento perfectos. Lo recomendaria sin duda alguna para cualquier cirugia similar.

After a laparoscopic or retroperitoneoscopic adrenalectomy you may notice some transient discomfort in your throat due to the plastic tube which was inserted into the trachea to administer anesthesia.

Because of the body's position in the operating theatre, you may also feel some discomfort similar to stiffness.

You will notice pain and a certain tension in the area of the scars, as well as a certain sensation of abdominal swelling if your surgery involved a laparoscopy.

These symptoms have a short duration of approximately 2 weeks and practically never need treatment.

The surgical procedure to remove an adrenal tumour does leave visible scars, but they are minimal.

Whenever possible, we will perform the operation by laparoscopy or retroperitoneal endoscopy. To remove this type of tumour we will need to make three to four incisions of approximately 1 cm.

You will probably be able to go home approximately 24-48 hours after the operation. On some occasions, the patient must be admitted for more days.

It is of the utmost importance.

Surgery of adrenal tumours must be performed by expert teams in Endocrine Surgery and at the same time experts in minimally invasive endoscopic surgery (laparoscopy and retroperitoneal endoscopy). This is the only way that an optimal result can be guaranteed with the patient's total recovery without side effects.

Soy médico. "Para mi es uno de los mejores cirujanos tiene unas manos para la cirugía que es de admirar yo tenía un diagnostico de cáncer en las tiroides y me hizo una intervención que no parece que he pasado nunca por una operación no tengo ni señal de la cirugía. Trato personal: Inmejorable tanto por su proximidad, afable, y transmitiendo una seguridad que hace que afrontes con total tranquilidad la operacion.

It is the removal of tissue from a lymph node to be examined under a microscope.

For safety reasons, it is preferably performed in the operating theatre and can be carried out in different ways:

- Open biopsy in superficial or accessible areas.

- Laparoscopic biopsy in accessible nodes of the peritoneal cavity.

The pre-surgical preparation includes a blood test, a medical evaluation, a chest x-ray and an electrocardiogram depending on the patient's age and medical condition.

A hernia is a protrusion of the contents of the abdominal cavity through the inguinal canal or another area such as the navel. When the inner layers of the muscles weaken, it appears as a bulge.

Hernias can appear in the inguinal region and also in other locations such as the navel or the lateral area of the abdomen.

Generally speaking, it is easy to recognise a hernia. You may notice an inflammation under the skin. You may feel pain when lifting heavy objects, coughing, or using force to urinate or defecate.

They occur very frequently and their repair is one of the most habitual surgical procedures.

A hernia does not improve over time, nor does it go away on its own. It can only worsen and impair the patient's quality of life, decreasing their mobility and independence.

After a thyroidectomy you may notice some short-term discomfort in your throat due to the plastic tube which was inserted in the trachea to administer the anesthesia: itching, pain when swallowing, a hoarse voice or even aphonia. Because of the head's position during surgery, you may even feel discomfort like a torticollis.
These symptoms only last for approximately 3 weeks and practically never need specific treatment.

Thyroid cancer may appear in a single nodule or may develop over a long period of time within a multinodular goiter. There are different types of malignant tumors in the thyroid, but the most common is differentiated thyroid carcinoma.

We can also find metastases from other cancers in the form of a thyroid nodule.

A thyroid cyst is an accumulation of fluid, which forms a pocket inside the thyroid. Most of these cysts are benign, although a small proportion can grow and cause significant discomfort. For this reason they must be analysed.

The usual treatment for a thyroid cyst is the aspiration of its content. However, in some cases, it is necessary to operate.

Invisible incision appendectomy

The surgery of acute appendicitis consists of making an 2.5 cm incision in the navel through which all the necessary instruments are inserted and the appendix can be extracted.

Performing the incision in this region provides a more comfortable post-operative period, since it reduces the risk of bleeding or bruising.

The reduction of three or four incisions to one, implies that the patient has less pain and that, as the scar remains hidden in the navel, also has a faster recovery.

Invisible laparoscopic cholecystectomy consists of the removal of the gallbladder using a camera and special instruments that allow the surgical procedure to take place through a single incision in the navel.

An adrenalectomy is a surgical procedure that removes one or both diseased adrenal glands.

There are several types of operations:
- Benign or malignant disease:

SINGLE INCISION TRANSUMBILICAL LAPAROSCOPY

Surgery through a 2.5 cm incision in the abdomen. During surgery, one or both glands can be removed. In certain cases, only the tumour can be removed, leaving the healthy gland part intact.
This is the ideal approach for tumours of> 4 cm on the left side.
This technique leaves a hardly visible scar.

STANDARD LAPAROSCOPY

Surgery through small holes (of 5 and 11 millimetres) in the abdomen. The gland with the tumour is extracted through one of the holes.
This surgical procedure is not very painful, allows the patient to recover quickly, has few complications and leaves very small scars.
At times, operating an adrenal tumor laparoscopically is not recommended because it is suspected that the tumour could be malignant or it is excessively large. In these cases an incision is made in the abdomen (traditional open adrenal surgery).

RETROPERITONEAL ENDOSCOPY

Surgery through small holes (of 5 and 11 millimetres) in the abdomen. The gland with the tumour is extracted through one of the holes.
This surgical procedure is not very painful, allows the patient to recover quickly, has few complications and leaves very small scars.
We use the retroperitoneal working space avoiding contact and manipulation of the intra-abdominal organs.
Despite it being a more difficult technique to perform, it offers obvious advantages for most patients; shorter hospital stay (48 hours approx.), less postoperative pain and a lower risk of abdominal complications.
It is the ideal approach for obese patients, patients with liver disease or people with previous abdominal surgery, since the peritoneal visceral organs are not put at risk.
It allows partial adrenalectomies to be carried out or cortical preservation in patients with bilateral tumours, patients who have been previously operated on or with MEN syndromes.

Most parathyroidectomies are performed through a 2 cm incision in the anterior and lateral part of the neck, practically without touching any other anatomical structure. This operation is selective and can be performed in 7 out of 10 patients. If it is necessary to locate the four glands, the incision will be central and somewhat longer.

In any case, whenever possible we take advantage of a neck wrinkle of the neck to hide the scar.

This operation is not very painful and most patients need only one to three doses of painkillers as post-operative care.

The gallbladder or cystic vesicle is a non-vital organ that aids digestion and stores the bile produced by the liver.

Sometimes gallstones or polyps can develop inside it.

If their existence is discovered accidentally with no digestion problems or alterations, it will only be necessary to perform routine controls. At times they can give the patient discomfort or symptoms that are attributed to other diseases.

If the gallbladder does not work properly or has stones inside it, it can give easily identifiable symptoms such as: colic pain in the liver region, indigestion, feeling of swelling in the stomach area, diarrhea and so on. If these symptoms are accompanied by nausea, vomiting, pain that does not subside or a fever, it could be a case of chronic colic or even an Acute Cholecystitis (an inflammatory process of the gallbladder).

The appearance of colicky pain, nausea, vomiting or fever is a warning that the gallbladder is not functioning properly and should be removed to avoid further complications.

The adrenal can stop functioning properly. There is a type of pathology that does not produce detectable hormonal alteration and therefore it can be difficult to diagnose, these would be non-producing tumours (adenoma, myelolipoma ...).

Usually this condition does not show any recognisable or specific symptoms, although patients may suffer from abdominal pain and discomfort or symptoms due to the compression of another organ.

Adrenal tumours are usually diagnosed incidentally (they are found when looking for another disease).

Another type of pathology is when a hormonal alteration ocurrs (defect or excess), such as adenoma, pheochromocytoma, Cushing's syndrome, etc.

The diagnosis is made through the study of these recognisable symptoms which are produced by the excess or deficit of the specific hormone.

It is necessary to remove the gland in the majority of benign non-functioning tumors of> 4 cm, and always in all benign functioning tumors (Conn's syndrome, pheochromocytoma) and cancer.

It is also frequent to discover metastases of other tumours that settle in the gland during the follow-up of the neoplastic disease.

Hyperparathyroidism is the hyperfunction of the parathyroid glands that leads to an excessive production of parathyroid hormone PTH.

Normally, this hormone excess is usually produced by a tumour or a growth of the glands as a whole. This causes the bones to decalcify and calcium to circulate through the blood in excess. When this happens, it is called a Primary Hyperparathyroidism (HPTP).

The symptoms that patients may suffer include headaches, irritability, osteoarticular pain, high blood pressure, osteoporosis, or even nephritic colics. As many of the patients are women, these symptoms are often wrongly attributed to menopause.

Goiter is a swelling of the thyroid gland in the form of a tumour in the front part of the neck. Sometimes this growth is invisible because it occurs inside the thorax.

Usually, thyroid hormone levels are normal and do not give any symptoms until the thyroid gland is significantly bigger.

It can cause respiratory distress (due to deviation of the trachea), especially when stretching in bed; nervous entrapment with irritative cough, changes in voice or aphonia and esophageal compression with discomfort and difficulty in swallowing solid foods or choking when ingesting liquids.

We must be aware that thyroid nodules larger than 3 cm should be removed, due to the compression they may cause and the risk of degeneration in thyroid cancer.

Hyperthyroidism is the term used to define the hyperactivity of the thyroid gland tissue, when an excess of circulating free thyroid hormones is produced: thyroxine (T4), Triodothyronine (T3), or both. These hormones are important at the cellular level and affect all the tissues of the body.

When it occurs acutely, it is called thyrotoxicosis.

The main function of thyroid hormone is the stimulation of metabolism and is essential for the normal function of cells. In excess, the metabolism is over-stimulated causing an "acceleration" of various systems of our body and resulting in symptoms similar to an overdose of adrenaline. These include rapid heartbeat and the appearance of palpitations, tremors and symptoms of anxiety. Diarrhea and weight loss can also appear.

A thyroid nodule is an abnormal growth of thyroid cells, which form a tumour inside the thyroid. The great majority of these nodules are benign. Nonetheless, unfortunately a small proportion are malignant and for this reason it is necessary to study them all.

Usually a partial thyroid excision is sufficient to treat a thyroid nodule.

Thyroid cancer comprises a group of four different types of cancer, of which the most common is Papillary Thyroid Carcinoma.

Between 5 and 15% of thyroid nodules are potentially malignant.

This cancer has one of the longest survival rates.

TYPES

Papillary Carcinoma: It represents 80-85% of the total of thyroid cancers. It grows very slowly. It is more prevalent in children and women of menstrual age. It is related to head and neck radiation exposure. Its extension is essentially local and lymphatic in the neck ganglia.

Follicular carcinoma: It represents 5% of the total of thyroid cancers. It grows very slowly. It is more frequent in menopausal women and in populations living far from the sea and with diets low in iodine. Its extension is fundamentally sanguine, mainly to the skeleton and to the lungs.

Medullary carcinoma: It represents 5-8% of the total of thyroid cancers. 25% have hereditary transmission. Its extension is fundamentally sanguine.

Anaplastic carcinoma: Very infrequent, <1% of cases. It grows quickly and has a very aggressive behaviour. More frequent in older people. Its causes are unknown.

Between 5 and 15% of patients undergoing thyroid surgery may suffer a transient recurrent nerve injury.

The surgeon's experience, technical aspects and the direct affectation of the recurrent nerve by cancer will directly influence the injury.

We have incorporated laryngeal nerve monitoring into thyroid surgery, allowing the intraoperative evaluation of the integrity of the recurrent nerve function.

This technology allows the recurrent nerve to be identified in those cases in which a difficulty in its location is foreseen (repeat operations, surgery for cancer recurrence, large goiters).

The surgical procedure to remove the adrenal glands can be done through two approaches: Transperitoneal or Retroperitoneal. We can offer either of these two approaches according to the patient's needs.

Transperitoneal laparoscopy (through the abdomen): We perform the intervention using minimally invasive techniques, both standard laparoscopy, mini-instruments as well as single-port surgery (SILS) for an impeccable aesthetic result.

There are patients in whom a standard laparoscopy can be difficult, problematic or even unadvisable to carry out due to their characteristics (obesity), their medical history (abdominal operations, stomata, abdominal visceral cancer, liver cirrhosis ...). In these cases, we are able to perform the procedure from "behind".

Retroperitoneoscopy (through the back): We perform the same procedure, but from the retroperitoneal space, avoiding contact and manipulation of the abdominal viscera. Despite being a more complex technique that requires more technical training, it offers obvious advantages for most patients, such as a shorter hospital stay, less postoperative pain and a lower risk of abdominal complications.

Dysfunctions of the adrenal glands can occur.

Usually adrenal tumours do not give any symptoms and so they tend to be diagnosed incidentally (they are occasionally found when looking for another disease).
Sometimes it is necessary to operate to remove the adrenal. This happens in benign tumours larger than 4 cm, all tumours that produce an excess of hormones (Conn's Syndrome, pheochromocytoma) and in adrenal cancer. At times metastasis from other cancers may also appear.

Adrenal cancer is rare but very aggressive. If cancer is suspected, it requires immediate surgery when the complete removal of the tumour can be guaranteed.

The adrenal glands are two endocrine glands (right and left) located in the depth of the abdomen, protected by the spine, ribs, kidney and abdominal viscera. Their primary function is to secrete a wide variety of hormones which are necessary to regulate body temperature, blood pressure, character, sleep-wake patterns, appetite, emotions and even sex. The importance of these glands lies in the fact that small hormonal alterations can produce serious disorders and diseases.

The procedure to remove the parathyroid glands does leave visible scars but they are minimal. Surgery is performed using the minimum incision necessary technique (MINP, minimal incision necessary for parathyroidectomy) or cervical endoscopy. Both of these approaches are carried out using microsurgical glasses and endoscopic support of the latest generation.

In many parts of the world, parathyroidectomy operations are performed with an incision of 4 to 6cm in the middle of the neck. However, we perform a mini-incision of 1.5 cm or several of less than 1 cm and the tumour or the diseased parathyroid gland are removed through these. They are very small incisions that do not leave any type of unsightly scar.

When the diseased parathyroid gland has been removed, the patient is cured.

Diagnosis of parathyroid diseases is simple. It is usually carried out with a detailed discussion and a routine blood test, which will show increased levels of calcium and PTH. Once we have these two analytical parameters, a cervical ultrasound and a cervical scintigraphy bone scan (SPECT-MIBI) should be requested to confirm the diagnosis and locate the problematic gland. If the gland cannot be located, other types of tests can be performed, such as an MRI or cervical CT.

When we have a PHPT diagnosis, the normal course of treatment is to operate. A parathyroidectomy operation is performed to remove the offending gland(s) and normally this is sufficient to solve the problem.

Parathyroid glands can suffer from disorders. Generally, a primary hyperparathyroidism (PHPT) is produced, this is a disease due to an excess of the circulating parathyroid hormone (PTH) which is produced by a tumour or a diffuse growth. The excess of hormone produced causes the bones to decalcify and calcium to circulate in the blood.

Usually this disease does not give any recognisable or specific symptoms, although patients may suffer from headaches, irritability, osteoarticular pain, even a nephritic colic, osteoporosis ... as the majority of patients are female, these symptoms are often attributed to menopause.

The surgery to remove the thyroid gland does leave visible scars but they are minimal. We perform the procedure using the minimal incision technique necessary (MINT, the minimal incision necessary for a thyroidectomy), making use of microsurgical glasses and the latest generation endoscopic instruments.

A normal thyroid measures between 5 and 8 cm. However, when there is a tumour, a cyst or a multinodular goiter in the thyroid, it can even measure over 10 cm. Traditionally, the thyroid can be removed with relative ease with a 8 to 10 cm incision. It must be remembered that the thyroid should be removed completely, without any fragments and with the capsule intact (in some cases the diagnosis of cancer is given by the thyroid capsule).

With the technique that we have developed, we can completely extract the thyroid with total safety, through a 3 to 5 cm skin incision in most cases. Therefore, even though the thyroidectomy is not completely invisible, it is true that it is such a small incision is almost imperceptible when it is performed in a skin fold of the neck.

There is a type of thyroid disease that does not produce detectable hormonal alteration and therefore it can be difficult to diagnose. This occurs in the case of a single cystic or solid nodule or a multinodular goiter (the whole gland grows in the form of multiple nodules and can produce an increase in the neck's diameter).

Another type of pathology is that which produces hormonal alteration (defect or excess), such as toxic sole nodules, hyperthyroid multinodular goiter, Graves-Basedow disease, thyroiditis, among others.
Although most of these dysfunctions can be treated medically, it is sometimes necessary to perform surgery on the thyroid. This is the case of some thyroid nodules, large multinodular goiters, hyperthyroidism (excessive functioning of the gland) and thyroid cancer.

Only thyroid cancer which is detected early and is well-operated on can be cured. That is why it is recommended that nodules> 3 cm are removed.

It is important that the surgical procedure is performed by an expert surgeon in order to guarantee the best prognosis of this disorder.
